We have particular experience supporting people to move from long-term hospital settings back into the community, taking time to plan each step carefully and reduce uncertainty along the way.
With support from our specialist team, including mental health input and Clinical Nurse Specialist oversight where needed, our focus is always on building independence, stability and long-term wellbeing.

Funding Support
We know the world of care and support funding can be overwhelming and difficult to navigate. We work with Integrated Care Board’s (ICB’s), Local Authorities (LA’s) and Case Managers directly to provide the personalised care and resources required for each individual.
