Providing Learning Disability support in England
We provide tailored support that helps people live the life they choose and achieve outcomes that truly matter to them. We work with individuals in both new and long-standing care arrangements, including supporting people to move from long-term hospital settings back into the community. Our support is backed by a multidisciplinary specialist team, including mental health expertise and Clinical Nurse Specialist oversight. Together, we focus on building independence, confidence and stability, supporting each person to develop skills, make choices and reach their full potential.

What we do:

The Learning Disability Care Package
We see each person as an individual first, and our support is shaped around that belief. We work closely with people and those important to them to design specialised care packages that reflect personal circumstances, strengths and aspirations, whether support is new or long established. Our focus is on achieving outcomes that truly matter to the individual — supporting choice, building independence and helping each person live the life they choose and reach their full potential.
Accommodation Support
Finding the right place to call home plays a big part in feeling safe, settled and supported. We offer flexible accommodation options shaped around individual needs, including support within the family home, an individual’s own home, single-person services and shared supported living.
We work closely with each person and those around them to help find the most suitable place to live, making sure the home environment supports independence, stability and the delivery of a tailored care package shaped around personal goals.

Clinical & Therapeutic Care
Effective learning disability support works best when therapeutic support and clinical care are carefully balanced. We connect people with our specialist support team, who provide mental health support shaped around individual needs. Where additional health support is needed, Clinical Nurse Specialist oversight is built into the care package, helping ensure needs are managed safely and consistently. By working in this joined-up way, we provide coordinated, seamless support that reduces pressure on families and focuses on outcomes that truly matter to the individual.
I had a great time Ice skating with my support workers. I was so pleased they were both able to take me. Going on the ice gave me the feeling of speed that I love. I’ll definitely be going again next year!

Funding Support
We know the world of care and support funding can be overwhelming and difficult to navigate. We work with Integrated Care Board’s (ICB’s), Local Authorities (LA’s) and Case Managers directly to provide the personalised care and resources required for each individual.
