Finally home, where he belongs!

Ayaan is a happy, bright three-year-old boy who has shown remarkable strength from the very beginning of his life. Born prematurely, he returned to hospital just three weeks after birth and spent more than 900 days receiving specialist care. In September 2024, Ayaan was finally discharged from hospital, marking an emotional and joyful moment for his family. For them, being together at home is everything.

Ayaan requires specialist care and is supported by a highly trained team of support workers. He is ventilated and relies on careful airway management to keep him safe. His team provides continuous observation, responsive airway support and tracheal suction when required, ensuring his comfort and safety at all times. He receives all nutrition and medication via a gastrostomy and is supported with personal care and hygiene throughout the day. Care is delivered with a strong focus on clinical safety, social wellbeing, and genuine compassion, allowing Ayaan to remain settled and secure in his own home environment.

Although Ayaan is not yet able to speak, he communicates in his own unique ways. Through facial expressions, clicking and raspberry sounds, he shows when he is content or unsettled. His support workers are skilled in recognising these cues and responding with patience, warmth and reassurance. Ayaan is partially deaf and wears an audio headband to help him explore sound, with his sensory needs supported through calm, engaging environments. After spending so much of his early life in hospital, being at home has opened up new opportunities for play, learning and connection. While his sister is at school, Ayaan’s support workers spend time playing with him, encouraging curiosity and interaction. He enjoys sensory toys, music and human connection, and thrives on time spent with his family.

Ayaan also enjoys getting out into the community, including trips to the park and visits to Acorns Children’s Hospice, where he takes part in activities in a warm, nurturing setting. Family sits at the heart of everything. With skilled support in place, Ayaan’s family can care for him at home with confidence and reassurance. His story is a powerful reminder that specialist care is not just about managing complex needs. It’s about enabling children to be children, supporting families, and making home possible.

Living Life with Joy, Confidence and Support

Carly is a vibrant 24-year-old woman who lives in a purpose-built bungalow, next door to her family home. This setting gives her the ideal balance of independence and closeness to her family, something that has always been incredibly important to her. Born with complex medical needs, Carly has required 24-hour care throughout her life. She has quadriplegic cerebral palsy and curvature of the spine, and is supported by a skilled, dedicated team of support workers who know her well and support her to live life to the fullest.

Carly’s care includes specialist support for epilepsy, with daily medication and close monitoring to manage both focal and tonic-clonic seizures. She also has asthma and receives respiratory support as part of her daily routine, including airway clearance interventions to support her breathing. Carly receives all nutrition and fluids via a mini-button, with her support team managing feeding, medication and personal care with skill, dignity and compassion. This consistent, attentive care provides reassurance for both Carly and her mum.

Those who support Carly describe her as an absolute joy to care for. She has a brilliant sense of humour, loves a bit of gossip, and brings warmth and laughter to everyone around her. Her mum shares, “Carly has an amazing life. Her team of support workers are like family.” That sense of closeness and trust shines through in everything they do together. Carly enjoys living life her way. She loves being pampered, with hand massages and foot spas helping her feel relaxed and confident. She enjoys shopping for clothes, going out for lunch, and attending a local day centre. At weekends, Carly takes part in wheelchair dance — an activity she truly loves, giving her the chance to express herself, socialise and enjoy movement in a fun, inclusive environment.

Recently, Carly and her mum went on their first holiday together to Blackpool. From accessible travel to overnight support, Carly’s team made it possible for them to enjoy this special time safely, confidently and full of laughter. Carly’s story shows that great care is about more than meeting complex needs. It’s about enabling joy, independence and meaningful life moments, and supporting people to live life exactly as they choose.

Living Life together, with Purpose and Independence

Over a decade ago, Moira’s life changed in an instant. A fall from her horse resulted in a life-changing spinal injury, leaving her tetraplegic with limited movement in her arms and legs. After a long period in hospital, Moira and her husband Colin faced a time of adjustment as they learned how to move forward together. Today, Moira lives independently in her own home with Colin, supported by a dedicated team of support workers providing care around the clock. While her physical circumstances have changed, her determination to live life as fully and normally as possible has never faded.

Moira’s support team works closely with her to ensure she feels safe, comfortable and in control of her daily life. Support is shaped around her routines and preferences, helping her manage personal care and day-to-day tasks with dignity and consistency. This reliable care gives both Moira and Colin reassurance, allowing them to focus on living rather than managing her condition.

Each day, support workers help her use an exercise machine that allows her to move both her arms and legs. Moira and Colin also enjoy swimming together in their indoor pool, with a support worker nearby when needed, enabling them to share meaningful time safely and independently as a couple. With a purpose-built vehicle, Moira and Colin can visit friends, go out for lunch and remain connected to their community. One detail mattered deeply to Moira: being able to sit in the front passenger seat next to her husband, just as she always had.

Colin admits he initially found having carers in their home difficult. “I really struggled in the beginning, but now I couldn’t do it without them”. The support team has become a trusted and respectful presence in their lives. Family remains at the heart of everything Moira does. She and Colin regularly see their two grown-up daughters, and Moira continues to stay connected to her lifelong love of horses. For Moira and Colin, what matters most is continuing to live life together. With the right support in place, Moira’s life is not defined by care, but enriched by it.

Safe, Supported and Independent at Home

Gavin spent 15 years living within secure hospital settings. He has learning difficulties, Autism, a mental health diagnosis, and is non-verbal. Gavin also experiences a significant intolerance to people entering his personal space, which requires careful, thoughtful support. Gavin is independent with his personal care, including washing and dressing. He receives 24/7 support on a 3:1 basis to help him manage his behaviours that can be challenging for others, ensuring he feels safe, understood, and respected at all times.

Gavin joined acuity care in 2021, as he began his transition out of a secure hospital environment. He moved into an NHS property, which became his home for 3 years. This setting enabled Gavin to live as independently as possible.In 2025, acuity care proudly supported Gavin to take the next step in his journey, transitioning from a robust NHS setting into a traditional bungalow located in a village community. Gavin has embraced his additional space and freedom his new home offers. He enjoys spending time in his new vehicle, benefits from increased privacy, and now has regular visits with his family. Gavin’s journey reflects acuity care’s commitment to providing compassionate person-centred support that promotes independence, dignity, and a true sense of home.

Supporting Independence and Improving Mental Wellbeing

A.D is someone we are proud to support. Before moving into her own home, A.D spent 14 years in a mental health hospital, until the service eventually closed. This individual has a history of complex mental health needs, including difficulties with emotional regulation.
After such a long period of time in hospital, transitioning into the community required careful planning and the right support in place.

Living in her own property has given A.D the opportunity to experience independence while maintaining stability and consistency in her daily life. Her home meets her practical needs and provides a safe, comfortable environment where she can feel secure.

Since moving into her own home, A.D has been building confidence step by step. With the support of her team, she is gradually becoming more comfortable with new experiences and activities.

This person we support is now engaging more with her local community and enjoying activities that were not possible during her time in hospital. She particularly enjoys going out locally, to coffee shops and shopping. Recently, A.D loved a trip out to the seaside, where she enjoyed having fish & chips by the sea.
These experiences play an important role in improving wellbeing, helping her to build positive routines and enjoy meaningful activities outside of her home.

A.D’s move into her own property has been a successful mobilisation, demonstrating how the right accommodation combined with specialist support can help individuals move from long-term hospital settings into the community.

In hospital, she said “I like to go for walks with other people because the outside world scares me. I do go out on my own sometimes, but I only go into the hospital grounds”.

Today, her life is very different from the one she experienced in hospital. She now has more choice, independence and opportunities to enjoy everyday experiences. Her journey shows how the right environment and the right support can transform a person’s quality of life and significantly improve their mental wellbeing.
